First, know this: the operation is less harmful than you think

If the size dropped noticeably, re-encoding took place. That means either the codec inside was incompatible with MP4, or compression was deliberately selected in the settings. Not all of the reduction is loss — modern codecs compress more efficiently — but there is some genuine detail loss as well.
If the size increased, a bitrate higher than the source needed was usually selected. This does not improve the picture, because extra bitrate cannot bring back detail that was never in the source; it only makes the file unnecessarily large.
Renaming the extension does not work
Because MOV and MP4 are related formats, changing the extension there sometimes produces a result. With MKV there is no such luck. Matroska has a completely different structure from MP4; turning a .mkv extension into .mp4 does not make the file an MP4, it only misleads the player and will most likely refuse to open at all.
